Rupture of bursting disk on charge vessel
Bursting disk on charge vessel found to have blown (not fatigued or faulty)
Vessel had previously held POCl3 before being washed through with DCM. Subsequent addition of aqueous sodium bicarbonate led to a small but signoficant reaction which evolved enough gas (CO2) to challenge the disk.
Decontimination procedure now improved, plus bicarbonate charged via alternative route.
