Common vent causes incident

A vent header was shared with other vessels, in one vessel was a monomer, the HCL scrubber became blocked, and a control valve in the vent line was wrongly set.   This led to some HCl gas being introduced to the monomer, which set off the polymnerisation, there were fume emmissions and the agitator sheared.
