Peroxide decomposition from vessel change

Plant experienced a rapid decomposition in a header.  The original plant had two headers, one for 27% peroxide and the other for strong caustic soda, this was later changed to a plant with one header only, to dose both liquids in sequence.   The alkali residue from the first dosing cause the peroxide dose to decompose.
