Poor PTW leads to Peroxide Decomposition

Eight tonnes of an organic peroxide was being stored in a cold room at -5°C.   A maintenance contractor decided to defrost the system, following the defrost procedure, he turned the compressor back on.  However the fan was NOT switched back on, and this resulted in the cold store warming up.  As this was a holiday weekend, the omission went unchecked, and the peroxide decomposed.
