Incorrect material charged to vessel
During a routine production the operator charged the wrong amine to the reactor. There was no incident, as such, just the cost of a lost bach and starting materials.
Two amines were being used in the plant, but in different bays. Although the drums were colour coded, the wrong material was added
- Human error can never be completely eliminated, only reduced.
- Unsegregated plant can throw up such errors as materials are close in proximity.
